How we handle information in your puppy application.
Information you provide
The application asks for your name, email, phone, mailing address, country, puppy interest, veterinarian contact details, household and pet information, an animal abuse/cruelty conviction yes/no answer, care and purchase acknowledgments, intended use, and a typed signature and date. Optional permission to call/text is recorded separately. Please do not provide payment information, identification numbers, or medical details.
How it is used
Your details are used to review your application and contact you about it. Applications, including sensitive answers, are stored privately in WordPress and accessible only to authorized site administrators. Answers are reviewed by Cindy and are not used for automatic approval or rejection. Notification failure status is visible to administrators. The form version and the policy/warranty text displayed at submission are retained with each application. Notification emails contain a sign-in link, not your application details.
Spam protection
The form uses a security token, a hidden spam field, and a temporary submission limit. A keyed hash of your network address is used for the limit and expires after one hour. Web hosting may separately retain access logs.

Cindy must approve a retention schedule and confirm the hosting, email, backup, and privacy practices for the live website. This local development site is for testing; use fictional application details.
